Service Coordination - Outreach Case Management |
The mission of the Outreach Case Management program is to ensure that clients can identify the necessary resources that correspond with their mental and physical health concerns.
The Outreach program is based on a more personal angle of evaluation through home visits and direct assistance for the homebound. The program identifies personal needs and refers to necessary services provided in the county.

Outreach Eligibility-

Clients must have difficulty leaving their homes. We believe that a client's inability to leave their home should not prohibit his/ her ability to access services.

Outreach clients should have concerns regarding any of the following: medical treatment, physical disabilities, prescription medication management, daily living skills, psychiatric symptoms, behavioral difficulties, housing instability, or homelessness.
