Healthworks
A grant based program to oversee the maintenance of health care delivery for children in the legal care and custody of the Illinois Department of Children and Family Services (DCFS) and who are placed in substitute care families located in Kendall County.

Obtaining an initial health screening (If not completed before placement)
Selecting a primary care physician
Scheduling a comprehensive health evaluation within 21 days of temporary custody
Obtaining routine medical examinations, including immunizations
Taking children for follow-up medical appointments or referrals for further medical assessment, treatment or services
Informing the child's caseworker, of medical problems or the need for medical services, as they occur
Making the child's Health Passport available to all medical providers at at each health visit
Bringing the child's Health Passport to each Administrative Case Review

Foster parents must contact the Health Department as soon as a child in DCFS custody is placed in their home. The purpose of this call is to select a primary care physician in order to schedule a Comprehensive Health Evaluation. The child's caseworker must provide the foster parent with an Encounter Form and Medical Consent to take to the Comprehensive Health Evaluation.

Foster parents can call the Kendall County Health Department (640) 553-9100 to obtain referrals for needed health services or providers.

Family Case Management
A program offering referral and resources to pregnant and postpartum women and newborn infants through the first year of life. Services include:

Prenatal and Infant Education- teaching mothers how to stay healthy during pregnancy, how to be ready for labor and delivery, and how to keep new babies healthy with well child visits and immunizations.
Health Insurance- assistance with obtaining access to health care with the State of Illinois All Kids program.
Ongoing support- one person is assigned to each pregnant woman and mom to provide ongoing support and answer question until the baby's first birthday.
Basic needs- we give referrals and directions to other programs so families can get items they need to be safe and healthy.

Teen Parent Services
The goals of this program are to assist young parents in getting information, services and skills needed to obtain and maintain mental and physical health for themselves and for their child. Participants receive parenting education, are encouraged to plan for their futures (by staying in school or completing a GED), and are enrolled in WIC and Family Case Management.

Eligibility:
Pregnant or parenting teen, under 19 and a half, working toward a diploma or GED, and receives Temporary Assistance to Needy Families (TANF) or All Kids (Medicaid) or is enrolled in Family Case Management or WIC.

Lead
Provide education, resources and referral, and follow up for residents of Kendall County with lead poisoning prevention questions or concerns. Coordinate care for children with elevated blood lead levels.
