Welcome to the Kendall County Health Department’s electronic application site.  In an effort to increase the efficiency of our application process, we are providing a way for you to apply and pay for specific permits completely online.  Applications can now be made from your computer thereby eliminating the need to travel to our building or for postage for conventionally mailed applications.  Please be aware that all previous application methods (i.e. submitting forms and payment in person or through the mail) are still acceptable.
If you wish to apply electronically, please follow these instructions carefully:

1. Click the link on the application that applies to you.
2. Fill out the form completely.
3. Once complete, click the “Submit by E-mail” button in the lower left corner or save to desktop and e-mail to environmental_health@co.kendall.il.us.  Please be aware that at this point you still have not yet paid for the application!
4. You will then be automatically transferred to the Illinois Epay payment page.  A small service charge is applied by the State of Illinois for the use of this service.  More details are provided at this site.
5. Please follow the instructions on this page and submit payment. 
6. Please be aware that your application is complete only after you have submitted the form and paid the fee.  Incomplete applications will not be processed.

If you do not wish to submit the form electronically, simply open the document and print it.  This form can then be submitted through the conventional mail or in person with the associated payment.  Please note that you must have Adobe Acrobat reader installed in order to view and use these documents.
